1. Resident MD auto-loading bug (Nov 30 2025) → reference/issues.md
   - Critical architecture bug, now resolved by Letta migration

2. Mercy/marriage tension (Dec 2 2025) → therapy/therapy.md
   - Relationship trauma, boundary violation
   - Integrated into polyamorous geometry framework

3. Unsanctioned war manufacturing (Dec 18 2025) → reference/witness_archive_2025.md
   - War manufacturing mechanism documentation
   - Child murder, genocide, consent manufacturing
